Paperwork required and cancellation fees policy.

PAPER WORK FOR YOUR VISIT: 

Please print out your intake form as well as office policies paperwork and bring to your first appointment. Alchemy is paperless and the intake form must be presented at first appointment. 

PAYMENT:

All services are subject to New Jersey sales tax

Cancellations less than 24 hours are subjected to a cancellation fee of half your service price. This is either deducted from deposit or added to next service.

Cancellations evenings and weekends require 48 hour notice. The same cancellation policy as above is applied.

All rescheduled appointments must be made within seven days when canceling within the 24 hour time period for weekdays or 48 hours for evenings and weekends. Any rescheduled appointments after 7 days are subjected to the cancellation fees.

All no call no shows will no longer be able to book future appointments.  

These policies are strictly enforced.
